Eagle Palm Advancement Form: A special form is required by the Scout Shop for Eagle Palms. Fill out the following form and bring it with you to your Scoutmaster Conference and Board of Review: EaglePalmForm
Medical Forms: Primarily needed for Summer
Camp, this form comes in two flavors:
Class1&2
for those of us lucky enough to be under 40 -- good for 2 years with update
Class
3 for the rest of us -- good for 1 year.
In addition, you may also need the Firearm
Permission Form
Tour Permit: For adults who would like to
lead any outing, this form is a must. It comes in two flavors:
Local
(under 500 miles) Best printed on Legal size paper
National
(over 500 miles) Best printed on Legal size paper

Merit Badge Counselor Application: Adult Merit Badge counselors are always needed. If any adult would like to volunteer to be a counselor for any or several of the many merit badge areas, please fill out and submit this form
Merit Badge Counselor List: A list of merit
badge counselors is maintained by the Scoutmaster and several of the Assistant
Scoutmasters of the troop. Please contact one of these leaders or send an
email to the Troop 1201 webmaster at the address at the bottom of this page with
your request.
Any scout parent may also obtain the list by filling out the request form at MB Cnslrs
Blue Cards are available on request from the Scoutmaster.
